Patient Forms
No one likes filling out forms, but...
...they are important so we ask you to do it.
It is important and real helpful if we get these forms before your appointment. This gives us time to feed our hungry computer before you are here so we can spend our time with you (instead of the computer).
There are two versions of each form:
1. Fill out online and send back to us electronically and securely. This is fastest. When you click on one of the online forms it will open a pdf that you can fill in ri9ght on your computer and send to us. Or it may, depending on your computer, download the form to your computer, where you can open it, fill it in and send it to us. Very simple.
OR
2. Download, print out, fill out and mail back. Obviously this way takes longer to reach us and it involves an envelope and stamp.
But either way is fine with us.
Please try for accuracy and completeness. Please fill in everything. The information is important and it truly helps us help you best and safely.
